77-Year-Old Mary Tafoya killed in Deadly Three-Vehicle Collision in Pueblo

The Pueblo community is grieving the tragic loss of 77-year-old Mary Tafoya, who died after a three-vehicle crash at the intersection of Northern Avenue and Prairie Avenue on December 1, 2025. According to Interim Pueblo County Coroner Gregory Grahek, the collision occurred around 3:48 p.m. and involved three vehicles traveling through one of the city’s busy intersections. She was transported to a nearby hospital, but despite extensive lifesaving efforts, she succumbed to her injuries at approximately 8:40 p.m.

While the identities of the other drivers have not been released, officials noted that everyone involved remained at the scene as the investigation began.

Pueblo police have stated that drugs and alcohol do not appear to have been contributing factors. However, investigators are examining other elements that could have played a role, such as speed, visibility, road conditions, or possible driver error. The intersection—known for heavy traffic flow throughout the day—was shut down for several hours as officers reconstructed the scene. It reopened by 8 a.m. on December 2 and is currently functioning as a four-way stop while authorities continue their work.
